Enter all information indicated by the red flag at the side of the page
- Section 1a) Enter your full name as the individual taxpayer or the name of your business (whichever that's applicable)
- Section 2) You can leave the taxpayer identification number blank if you do not have an EIN number, the payroll team will apply for this on your behalf.
- Section 6) Enter your full name as the contact person or your spouse's name
- Section 7) Enter your mobile or daytime phone number
- Section 15) Enter the current year and the quarter that you've hired your nanny, please see the information below to guide you with the quarter:
January to March, YYYY/03
April to June, YYYY/06
July to September, YYYY/09
October to December, YYYY/12
6. Section 16) Enter the year and date of the month that payroll begins, if your nanny should be paid in May, you can enter YYYY/05
7. Ensure you sign and date the document
8. SUBMIT
NOTE: You need to verify your document by logging into your email address and clicking on the verification email
