How to fill out my ACH authorization form

An ACH authorization form is a legally-required agreement to authorize the withdrawal from your bank account. If required, you can access the form here: ACH Authorization Form

Please follow the instructions to complete the form:

1. Legal Business Name: Enter your full name or Business (whichever is applicable)
2. Tax ID/EIN#: Leave blank if you don't already have one (the payroll team will apply for this on your behalf)
3. Business Line: Enter your home address 
4. Owner/Principal Name 1: Enter your full name 
5. Business Account for ACH transactions is your banking information
6. Acknowledge your name, sign, date and SUBMIT
